This contract opportunity is for a Home Health Physical Therapist supporting elderly patients through a specialized independence-focused program. The role involves delivering personalized physical therapy services during home visits within Mercer County and North Burlington County, NJ, Monday through Friday from 8:00 AM to 4:30 PM, with no weekend or on-call duties.

Key Responsibilities:

Perform comprehensive patient evaluations and develop customized treatment plans.
Provide therapy to enhance mobility, functional abilities, and overall quality of life.
Educate patients and caregivers on therapeutic exercises and home care techniques.
Collaborate with physicians, nurses, and case managers to optimize patient outcomes.
Maintain precise and timely clinical documentation.

Qualifications:

Graduate of an accredited Physical Therapy program.
Licensed Physical Therapist in the state of New Jersey.
Current AHA Basic Life Support (BLS) certification.
Minimum of 2 years’ experience working with adult geriatrics.
Proven experience in home health and senior care settings.
Strong communication, documentation, and critical thinking skills.
Reliable transportation necessary to travel to patient homes.

Preferred:

Experience in pelvic floor therapy.
Training in Herman & Wallace Level 1A or an equivalent certification.

Physical Demands & Work Environment:This role requires frequent standing, walking, and driving, with occasional lifting up to 50 lbs., bending, and reaching during patient care.
